does registered rep need to report every trade in his/her own accounts?

It depends on the policies of your broker/dealer. There is not any specific securities regulation that requires trades be disclosed, but the broker/dealer is required to be in compliance with various trading rules (including trading ahead of clients or insider trading) . I have worked at firms that actually required prenotification of all trades and at firms that only required a copy of quarterly statements.

I think if I’m not mistaken, yes you need to report it to who ever is holding your license. Otherwise they could get into trouble.
